The necessary documents that must be included with the application form
The application must be submitted with copies of the following certifications, each properly attested by a Gazetted officer. The application should not be provided with original certificates.
- Certifications of education and professional proficiency.
- A duplicate of your birth certificate or your class 10 diploma.

Medical Specialist
In the relevant specialty, MD/MS.
