Important Documents for Filing Forms
A properly self-attested photocopy of each of the following documents or certificates is required to be attached to the application:
(a) The grade report for the required foundational coursework.
(b) A certificate of graduation from school, a certificate of municipality birth, or a certificate of school departure.
(c) One envelope that is self-attested and properly sealed with a postal stamp of Rs. 5/-.
(d) A current passport-size photo, properly confirmed by the subject on the reverse of the image.
(e) Discharge book, if applicable, or matriculation certificate in the case of ex-servicemen, properly attested by a Zilla Sainik Welfare Officer, PPO, and AEC-II or II.
(f) A job registration card, should one exist?
(g) Any applicable experience certificate.
(h) For reserved positions, a photocopy of the caste certificate (SC/ST & OBC (Non Creamy Layer)), is properly attested by the individual.
(j) A copy of the medical certification or disability certificate from the civil surgeon at the Govt Hospital or CMO, if the candidate is applying for PHP.
(k) “No Objection Certificate” (NOC) for Central Government workers in service to receive age relaxation benefits.

Question Papers
All written test question papers will be multilingual (in Hindi and English).
The following is the syllabus for the written test used to fill the position of LDC:
Two hours are allotted.
Paper topics include the following:- (A) All-around intelligence.
Language proficiency in English (b).
(c) The ability to count.
(d) Broad Awareness.
(f) A computer will be used to administer the skill test.
The following is the curriculum for the written test required to be considered for the other position:
Two hours are allotted.
20 minutes of additional remuneration for the PHP candidate.
Type of Question Paper: Objective.
Each incorrect response is penalized with 0.25 marks.
The paper’s topics are as follows:
(a) 25 questions, 25 marks for general intelligence and reasoning.
(a) 25 questions, 25 marks for numerical aptitude.
General English (c): 50 questions, 50 marks.
(c) 50 Qs, 50 Marks for General Knowledge.
